(*) Form S1 is a document that you must request from your health insurance authority in your country before you leave. It entitles you to sign up to the social security system in France. Form S1 is only available for EU/EEA/Swiss citizens.

Family

  • You and your family’s vaccination and health records. The vaccination records are required for enrolling in school.
  • Your children’s school report cards if they are accompanying you.

PLEASE, NOTE! Bring original copies of the documents mentioned but keep them safe and never hand them over unless at the explicit request of an administrative body.

Make sure you send clear, legible and full copies for any administrative procedure. The prefecture may refuse any truncated document which will slow down your application.
